Young Adult Carers (YAC) Support unpaid carers within Carers Trust Crossroads West Wales.
The Children & Young People’s Service works with carers aged 5-25 through conducting assessments, care planning, 1:1 keywork sessions, multi-agency work, regular youth clubs, days out, residential trips and life skills workshops.
